1. Prepare your server

1. Create a channel for the menu where users open forms.
2. Create a log/target channel where submissions should be sent.
3. Make sure the bot can view and send messages in both channels.

2. Configure in panel

1. Log in and choose your server.
2. Create a form and define its fields.
3. Set the target channel or enable ticket-based mode.
4. Create a menu and connect buttons to forms.

3. Send the menu

1. Send it from the panel or via /menu send key:<menu-key>.
2. Users can then open and submit forms directly from Discord.

Note: Discord modals support up to 5 interactive components per modal.
